While the systemic damages of anabolic-androgenic steroids (AAS) affect everyone, introducing synthetic male hormones into the female body system triggers distinct physiological failures. Because a woman’s baseline testosterone is naturally low, exposing her organs, bones, and muscles to supraphysiological male hormone levels causes severe, often irreversible disruptions.

There are even troubles with your tendons and many more problems later in life that will affect your whole body.

1. The Endocrine & Reproductive Systems: Irreversible Virilisation

The female endocrine system relies on a delicate balance of estrogen and progesterone, regulated by the Hypothalamic-Pituitary-Ovarian (HPO) axis. Introducing synthetic male hormones entirely derails this system.

  • Virilisation (Masculinization): This is the most profound side effect for women. Unlike men, who may recover some natural function after a cycle, many masculine changes in women are permanent because the tissues physically alter. These include a permanent deepening of the vocal cords (deep voice), clitoral enlargement (clitoromegaly), and male-pattern hair growth (hirsutism) on the face, chest, and back, alongside male-pattern baldness.

  • Ovarian Shut Down & Infertility: The hypothalamus detects high androgen levels and stops signalling the ovaries. This causes an immediate cessation of the menstrual cycle (amenorrhea), halts ovulation, and can result in long-term or permanent infertility, alongside uterine atrophy (shrinking of the uterine lining).

  • Severe Estrogen Crash: Dropping estrogen levels to near-zero mimics sudden, premature menopause. Women frequently experience hot flashes, severe night sweats, extreme mood volatility, and chronic vaginal dryness.

2. Bones and Muscles: The Structural Breakdown

While steroids rapidly build muscle tissue, they paradoxically compromise the foundational structure supporting that muscle.

  • Early Osteoporosis Risk: Estrogen is the primary hormone responsible for maintaining bone mineral density in women. By suppressing ovarian function and lowering estrogen levels, long-term steroid use leads to premature bone thinning. This leaves female athletes at a much higher risk for early-onset osteopenia, osteoporosis, and stress fractures.

  • The "Engine vs. Chassis" Problem in Women: Because women generally start with less tendon cross-sectional area than men, the rapid muscle hypertrophy (growth) induced by steroids places immense mechanical strain on unadapted connective tissues. The muscle becomes far too powerful for the tendon to support, leading to severe chronic tendonitis or catastrophic tendon ruptures (such as the patellar or Achilles tendon) during heavy lifts.

3. Cardiovascular & Internal Organ Damage

Women naturally enjoy a strong cardiovascular protective shield thanks to endogenous (natural) estrogen. Steroids completely strip this protection away.

  • Cardiovascular Disease Acceleration: Estrogen helps keep blood vessels flexible and maintains healthy cholesterol. Steroids obliterate this benefit by dropping HDL ("good") cholesterol, raising LDL ("bad") cholesterol, and causing severe hypertension (high blood pressure). This shifts a woman’s cardiovascular risk profile to match or exceed that of a high-risk male, drastically increasing the lifetime chance of heart attacks and strokes.

  • Left Ventricular Hypertrophy (LVH): The muscle tissue of the heart thickens pathologically in response to the massive stress of heavy lifting combined with high blood pressure and steroids. This rigidity limits the heart's pumping efficiency and can lead to arrhythmias.

  • Liver & Kidney Stress: Just like in men, filtering heavy loads of performance-enhancing drugs—especially oral steroids—causes toxic stress to the liver (hepatic adenomas, elevated enzymes) and forces the kidneys to filter excess metabolic waste under high pressure, leading to long-term renal scarring.

From a clinical sports massage perspective, working with female clients who use or have used performance-enhancing and steroids requires specialised awareness. They are highly susceptible to connective tissue injuries due to the sudden mismatch between muscle power and tendon strength, and they face a heightened risk of stress fractures due to suppressed estrogen levels.
